I hereby submit my registration for the workshop and accept that, by submitting this form to Kriek Design Consultants (Pty) Ltd., I am entering into a legal agreement to pay the correct amount to Kriek Design Consultants (Pty) Ltd. not later than seven (7) calendar days prior to the workshop or any other period as Kriek Design Consultants (Pty) Ltd. may stipulates from time to time.

I understand and accept that Kriek Design Consultants (Pty) Ltd. reserves the right to cancel or postpone this workshop at any time if there are not enough registrations received to make it economically viable or due to logistical reasons. An e-mail stating the cancellation or postponement of the workshop to the e-mail address provided below will be deemed sufficient notification.

I understand and accept that Kriek Design Consultants (Pty) Ltd. may ask for proof of identification of any delegate attending the workshop.

I understand and accept that CPD Certificates will be issued only to registered professionals if a valid registration number is supplied. Attendance Certificates will be issued to all others.

I understand and accept that I must download the CPD / Attendance Certificate from www.saiat.org.za and that the certificate will not be e-mailed to me.
